Offshore Fishing Safety: The Five Things That Actually Go Wrong

The trouble with a gear list is that it doesn't tell you anything about how trips actually go sideways. Nobody gets hurt because they were missing an item from a list. They get hurt because something specific happened, usually something ordinary, and they hadn't thought it through beforehand.

So here are the five things that actually go wrong offshore, what each one looks like when it's happening to you, and what changes the outcome.

The weather turns

Weather is the most common way a good day offshore becomes a bad one, and the reason is simple: the forecast is a probability, not a promise.

Assume roughly even odds that conditions will be worse than forecast. Not because forecasters are careless, but because marine forecasts describe a broad area and an average, and you're fishing one specific piece of water at one specific hour. A forecast of two to three feet routinely delivers a four-foot set. Wind that was supposed to lay down at noon sometimes doesn't.

Plan against the pessimistic version. If the forecast is at the edge of what your boat and your crew can handle, the honest read is that the day is already outside your margin.

Once you're out, keep watching. A VHF with weather channels, a satellite service, or radar on your phone while you still have signal โ€” any of them beats looking up and being surprised. Watch the sky to your west and the barometer if you have one.

The part people underestimate is the ride home. Getting offshore in a building sea is easy because you're running with fresh fuel, fresh crew, and adrenaline. Coming back in the same sea, tired, with a following sea pushing your stern around, is a different boat entirely. Build the return trip into your decision about when to leave, and leave earlier than feels necessary.

The engine quits

An engine failure inshore is an inconvenience. Thirty miles out, it's the beginning of a long day, and how long depends almost entirely on what you did before you left the dock.

An anchor that actually holds

A disabled boat drifts, and it usually drifts away from where anyone is looking for you. Enough rode to hold where you fish is worth more than most of the electronics aboard.

Reaches other boats and the Coast Guard, and the distress button transmits your position with the call. Cell phones work until they don't, and that's usually the moment you're furthest from a tower.

Costs less than a tank of fuel and turns a five-hundred-dollar problem into a phone call.

None of this prevents the breakdown. What prevents breakdowns is boring: fresh fuel, clean filters, a spare, and paying attention when the engine starts sounding different. Most failures announce themselves before they happen. The skill is being willing to turn around when you hear it.

Fire

Fire is the hazard almost nobody plans for, and it's the one that gives you the least time to react.

Gasoline vapor is heavier than air and collects in the bilge, where it sits waiting for a spark. This is why you run the blower before starting, and why you do it every time rather than when you remember. It's also why fueling procedure matters: engine off, everyone off the boat, nozzle in contact with the fill, and a sniff of the bilge before you hit the key.

The other common source is electrical. Corroded connections, an overloaded circuit, a chafed wire behind a panel. Salt air works on wiring constantly, and a connection that's slowly turning green is a connection heating up.

Two questions worth answering before your next trip

Can you reach your extinguisher from the helm without going through the fire? An extinguisher stowed in the cabin is useless if the cabin is what's burning.

When was it last checked? Extinguishers lose charge sitting in a hot boat. Look at the gauge. Most people find out theirs is dead at the worst possible moment.

Offshore, a fire you can't put out in the first minute or two means you're going in the water. That makes the next item much more serious.

Hazard 04

Going overboard

Falling off the boat is survivable if someone stops the boat. If you're alone, nothing stops the boat.

That's the whole problem in one sentence, and it's why the engine cut-off switch is the most important safety device most anglers don't use. A fall over the side kills the engine instead of leaving you watching your boat motor toward the horizon. In the U.S., operators of boats under 26 feet are required to use the cut-off link in most underway situations. Compliance in the real world is low. Solo, it's the difference between an embarrassing swim and a fatal one.

Why the lanyard doesn't get worn, and what fixes it

Be honest about why that coiled cord sits unused on most boats: it ties you to the helm. You clip in to run, then unclip to work a fish, clear a line, or reach the transom. Unclip once and it generally stays unclipped for the rest of the day โ€” which is the exact stretch of time you're moving around a wet deck.

Wireless cut-off systems remove the tradeoff. You wear a small transmitter on your wrist, belt, or PFD, and a receiver hub wires into the boat's 12V or 24V system in parallel with the existing ignition circuit, so the original lanyard still works as a backup. If the fob goes out of range โ€” typically around 50 feet โ€” or gets submerged, the engine shuts down within a second or two.

The part worth knowing if you fish with company: most systems pair several tags at once. Everybody aboard wears one, and any of them going over the side triggers an alarm or kills the engine. That may matter more with a crew than alone. A quiet man-overboard while three other people are watching rods is exactly the kind of thing nobody notices until the spread comes up short.

Installation is easier than people expect. These systems are designed for the owner to install, not the boatyard โ€” the hub ties into the existing ignition cut-off circuit, the kits come with the connectors and a wiring diagram, and basic tools get it done. I put mine in myself in an afternoon. It's a one-time job that converts the cut-off switch from a rule you ignore into a device that works whether you remember it or not.

An inflatable PFD is the other half. Modern suspender-style vests are comfortable enough that you'll actually keep one on, which matters more than the rating on the label. Auto-inflating models deploy on contact with water, which is the version you want if there's any chance you're unconscious when you hit.

Go check this on your own boat

Can you get back aboard from the water, by yourself, in the gear you're wearing? Freeboard that looks modest from the deck is a wall from the surface. Many boats have a ladder that only deploys from above. If yours does, fix it โ€” a rope loop, a fold-down step, anything you can reach while treading water.

If you fish alone, add a personal locator beacon on your person, not in a bag. An EPIRB mounted on the boat is excellent for a sinking. It does nothing for you if the boat is a quarter mile away and still running.

Hazard 05

Cuts, punctures, and what grows in them

This is the injury you'll actually experience, probably more than once, and it's the one people treat most casually.

The sources are ordinary. A bait knife on a wet, moving deck. A treble hook in a green king mackerel that isn't finished fighting. Fingers near gills. Wahoo and king mackerel have teeth that will take you to the bone without much effort, and both are usually still angry when they come over the rail. Mahi thrash. Spines and dorsal rays puncture. Gaffs slip.

Most of these are manageable in the moment. Pressure, clean it, close it, keep fishing.

The part that isn't obvious is what happens over the next two or three days.

Warm saltwater carries bacteria that don't behave like the ones you're used to. Vibrio species live in coastal salt water and can turn a minor cut into a serious infection remarkably fast โ€” hours, not days. Mycobacterium marinum, sometimes called fish handler's disease, comes from fish slime and puncture wounds and produces a slow, stubborn infection that ordinary antibiotics often miss because nobody thinks to look for it.

The practical version

Irrigate cuts with clean fresh water as soon as you can, and do it generously. Rinsing matters more than what you rinse with.

Puncture wounds are worse than they look. A treble hook or a spine drives bacteria deep and then closes over the top.

If a wound gets red, swollen, hot, or painful out of proportion to its size, go in. Same day. Say you were handling saltwater fish โ€” that sentence changes what a doctor tests for and what they prescribe.

People with liver disease, diabetes, or a compromised immune system face substantially higher risk from Vibrio. If that's you, treat every saltwater cut seriously.

Carry a real kit: sterile saline or a squeeze bottle for irrigation, gauze, self-adhering wrap, closure strips, gloves, and something for a deep bleed. Bandages and antiseptic wipes are for paper cuts.

Bottom line

The point of all this

None of the five is exotic. Weather, breakdown, fire, a fall, a cut. Every one of them is ordinary, which is exactly why they're worth thinking about now rather than at the moment they happen.

File a float plan before you leave โ€” departure and return times, where you'll be, a description of the boat, who to call. It costs two minutes and it's the reason anyone knows where to start looking.

Then go fishing. Preparation isn't about expecting the worst. It's about being free to stop thinking about it.
